Will the enrolment log be returned to you as it is or after removing the identifiable NHS fields?

You will need to remove the identifiable fields before returning to us- there is a separate tab at the bottom that automatically copies the data need over so you can just copy and paste it into a new document for us. This will be explained at the SIV meet
